Terms & conditions for use of Parent- leave no child indoors- Questionnaire® (henceforth termed The Questionnaire) ​​

 

Copyright Ownership 

The Parent- leave no child indoors Questionnaire® is the intellectual property of Professor Michael Chia c/o Physical & Education & Sports Science, National Institute of Education, Nanyang Technological University, SINGAPORE. All rights are reserved. No part of the Questionnaire may be reproduced, distributed, or transmitted in any form without prior written permission from the copyright holder.   

 

Permitted Use 

The Questionnaire are available for non-commercial research, educational purposes, and program evaluation only. Users must acknowledge the copyright owner in any publications, presentations, or materials derived from or referring to the Questionnaire.   

 

Prohibited Use 

The Questionnaire may not be modified, adapted, or translated without prior written approval.  Commercial use, including resale or incorporation into for-profit training or products, is strictly prohibited unless explicitly agreed upon in a separate licensing agreement.   

 

Authorization for Use 

Prospective users must seek written email approval via detailing: Purpose of use, Research/project scope, and Anticipated outcomes. Email approval will be provided on a case-by-case basis.   

 

Distribution and Sharing 

Users are not permitted to share, distribute, or post the Questionnaire online or in any publicly accessible forum.  Any redistribution must have written approval by the copyright holder.  

 

Attribution 

Publications or presentations using data or insights from the Questionnaire must include the following citation:

Updates and Amendments. The copyright holder reserves the right to update the Questionnaire and these terms and conditions. Users must comply with the most recent version.  

 

Termination of Use 

The copyright holder may revoke permission via written notification if the terms and conditions are violated. Users must cease all use immediately upon notification.
